Check your current agreement
Check the cancellation requirements and notice period in your current Service Agreement.

Sign your
documents
Your new Service Agreement and Participant Consent Form will be sent to you to sign and return.

Let your current plan manager know
Tell your current plan manager in writing that you are changing providers and ask them to confirm your final date.

Update your
NDIS details
Tell the NDIS that you are changing plan managers and complete any required endorsement.

Register
online
Complete The Good Plan Manager’s online registration form.

Let your
providers know
Give your providers The Good Plan Manager’s invoice-submission details.

I’m writing to let you know that I’d like to change plan managers.
Could you please confirm my final date and let me know if there are any other steps I need to complete?
Please also arrange for any outstanding invoices to be forwarded when the change takes place.
